Click below to download and fill out the application appropriate to your child’s age level. Our Toddler program is for students 18 months and older. Students must be 18 months old by Sept 1st to apply for the Toddler Program. Students must be 3 years old by September 1st to apply for the Primary program, 5 years old by September 1st to apply for Kindergarten, and 6 years old by September 1st to apply for first grade.
Applications can be returned to the school via email (kms@khalsamontessori.org), fax (602-252-5224), postal delivery (Elementary-2536 N 3rd Street Phoenix, AZ 85004) or can be dropped off at the Elementary front office in person.

Private School Tuition Rates
Khalsa Montessori grades Toddler through Primary are part of the private school and therefore have associated tuition.
